DESCRIPTION:It is now some seven years since the  nuclear accident at Fuku
 shima Daiichi.  Many reports have been written about it\, including a 1000
  page authoritative report by the IAEA\, and numerous lessons identified\;
  but\, have we got to the root cause of the problem and found a way of add
 ressing this lesson.  Professor Mike Weightman will explore this question 
 based on his own experience of helping the UK to respond in the immediate 
 aftermath\, leading the IAEA expert fact finding mission to Fukushima Daii
 chi in May/June 2011\, and helping the Japanese to decommission the site.\
