DESCRIPTION:I have over the last 40 years investigated a number of acciden
 ts\, mainly in the chemical process industries and specifically in the oil
  and gas industries.   \nOver that period\, my research has become almost 
 entirely focused on what is called blowdown\, that is the depressurisation
 \, whether accidental or deliberate\, of systems (vessels and pipelines) c
 ontaining high-pressure hydrocarbons.   \nIn this talk\, I will discuss ve
 ry briefly four of these accidents: Umm Said in 1977\, Ocean Odyssey in 19
 88\, Longford in 1998 and P36 Roncador in 2001.   \nI will then discuss in
  a bit more detail the accident on Piper Alpha in which 167 men died in 19
 88 (two months before Ocean Odyssey) and will draw some lessons that seem 
 to me to be common to many other accidents.
