DESCRIPTION:Traditional safety engineering techniques have limitations whe
 n applied to the complex\, software-intensive systems we are building toda
 y. In this talk\, we will describe a new model of accident causation and a
 pproach to engineering safer systems based on this model. An advantage of 
 this approach is that it applies not only to the technical aspects of syst
 ems but also to the larger social aspects\, such as programmatic risk and 
 safety culture. Various pplications in several application areas\, includi
 ng aerospace\, defence\, and pharmaceutical safety will be described.
