Abstract
Traditional safety engineering techniques have limitations when applied to the complex, software-intensive systems we are building today. In this talk, we will describe a new model of accident causation and approach to engineering safer systems based on this model. An advantage of this approach is that it applies not only to the technical aspects of systems but also to the larger social aspects, such as programmatic risk and safety culture. Various pplications in several application areas, including aerospace, defence, and pharmaceutical safety will be described.
