The implementation of pharmacogenomic knowledge has been slow, and the pace of such implementation has been disappointing to many. The barriers to implementation are multiple, and include scientific, financial, and technical barriers. Although the quantity of scientific publications in pharmacogenomics is increasing at an exponential rate, the quality of such publications is highly variable, challenging standard approaches to meta-analysis. Furthermore, most investigators have focused on common –rather than rare –variants. Financial barriers include the high expense of genotyping relative to standard laboratory tests, further complicated by limited intellectual property protection for pharmacogenomic tests. Technical barriers include physician knowledge, as well as time delays in obtaining results. One possible implementation solution is a genomic prescribing system, such as the one in development at the University of Chicago (http://cpt.uchicago.edu/page/genomic-prescribing-system-gps).
